Position Overview
The Client Care Coordinator plays a vital role in supporting our clients, families, and caregivers. This position serves as the primary point of contact for care coordination, scheduling, and client support. The ideal candidate is compassionate, organized, and thrives in a people-centered environment where relationships matter.
Key Responsibilities
- Serve as the primary liaison for clients and families, providing responsive and compassionate support.
- Coordinate and schedule caregiver assignments based on client needs and caregiver availability
- Conduct in-home assessments and assist with developing individualized care plans.
- Monitor ongoing care services to ensure quality, satisfaction, and continuity of care
- Respond to and resolve client or caregiver concerns professionally and empathetically.
- Maintain accurate and confidential client documentation in compliance with HIPAA.
- Support caregiver recruitment, onboarding, training, and retention efforts
- Participate in an on-call rotation to support after-hours needs as required
- Ensure compliance with Vermont state regulations and company standards
Qualifications
- High school diploma or equivalent required; Associate’s or Bachelor’s degree preferred
- Minimum of 1 year of experience in client services, care coordination, or home care
- Strong communication, organization, and problem-solving skills
- Compassionate, service-oriented, and detail-focused mindset
- Proficiency with Microsoft Office and home care software (ClearCare, AlayaCare, or similar preferred)
- Ability to manage confidential information with discretion and professionalism
- Valid driver’s license and reliable transportation required for home visits

- Expected Pay Range: $20.00–$25.00 per hour, based on experience, qualifications, and internal equity
