The Clinical Support Associate is a vital member of a multidisciplinary team responsible for the delivery of a variety of resident care services. COTS’ service approach is focused on effectively combining services that address mental health, physical health, substance abuse, social determinants of health, and housing readiness for COTS’ Shelter, Housing, and Homeless program clients.

The Clinical Support Associate ensures delivery of appropriate interventions and care coordination in line with CalAIM Enhanced Care Management (ECM) service requirements by providing clinical support and care coordination to non-clinical care managers and program managers - including clinical facilitation and guidance in case conference meetings, the development of Individualized Care Plans for clients enrolling in ECM services, and other clinical supports. This position also provides and assists with clinical care through one-on-one and group therapy sessions for COTS clients. The Clinical Support Associate reports to the Clinical Care Manager.

In addition, in partnership with the SAFE Team, the Clinical Support Associate provides 24/7 crisis on call support to our community on a rotating basis with other COTS clinicians in support of southern Sonoma County. Calls are not frequent and include supporting SAFE Team staff via telephone; on rare occasions in-person support may be needed. This support is assigned weekly for at least one week out of the month.

This position will receive supervisory hours toward licensure, provided by their clinical supervisor (LCSW), for a duration of 3 hours per week (one hour individual, 2 hours group).

ESSENTIAL DUTIES

  • Collaborates alongside the case management teams to ensure client care.
  • Reviews client intake assessments, using clinical judgment, to identify and address client needs in the areas of healthcare, mental health, and substance use disorders.
  • Coordinates with the Care Manager, the development of client’s Individualized Care Plans, based on intake assessments and client’s health goals.
  • Ensures delivery of appropriate interventions and care coordination of CalAIM Enhanced Care Management (ECM) services.
  • Holds a therapy caseload of up to 20 clients in COTS Shelter, Housing, and or Homeless programs.
  • Provides clinical trainings to staff as well as clients.
  • Provides clients with self-management skills and information needed so they can manage their health and housing readiness.
  • Attends weekly individual and group supervision.
  • Holds the crisis line for at least one week per month.
  • Participates in case conferencing meetings to review and evaluate the services provided to residents, and to identify opportunities to improve service delivery.
  • Completes required documentation and paperwork in a timely manner, and in accordance with COTS and funders standards.
  • Always adheres to HIPAA privacy and security requirement standards.
  • Ensures HIPAA privacy and security requirements and standards are being met by COTS staff and swiftly and thoroughly address non-compliance issues as they arise.
  • Other duties as assigned.

QUALIFICATIONS

  • Ability to work as a member of a multidisciplinary care team.
  • Experience with CalAIM Enhanced Care Management services is desirable.
  • Experience working with vulnerable populations, ideally individuals who have or are currently experiencing homelessness.
  • Experience with clinical case management is strongly preferred.
  • Must have some experience in providing individual, group, or couples counseling or can demonstrate a high level of knowledge regarding providing therapeutic services.
  • Experience with case conceptualization, treatment planning.
  • Understand the impact mental health, substance use, and trauma has on development and behavior.
  • Knowledge and experience working with co-occurring populations (chronic health conditions, mental health, and substance use).
  • Knowledge and experience with evidence-based practices, including trauma informed care framework.
  • Ability to provide trauma-informed care.
  • Proficient in Microsoft 365
  • Experience in an HMIS database preferred.
  • Cultural competency and commitment to clients.
  • Ethical and professional behavior.
  • Bilingual (English/Spanish) highly desirable.

EDUCATION
Master’s degree in social work from an accredited university required.

CERTIFICATES, LICENSES, REGISTRATIONS

ASW license with the California Board of Behavioral Science in good standing required. Valid Driver’s License, access to reliable personally insured vehicle, and acceptable motor vehicle report required.

LANGUAGE, MATH & REASONING SKILLS
Ability to read, analyze, and interpret documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations, including medication measurements. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in written or diagram form and deal with several abstract and concrete variables.

This position’s primary location is 900 Hopper Street, Petaluma. Telecommuting up to one day per week may be optional depending upon client, colleague, and supervisor needs.
