COTS service approach is focused on effectively combining services that address mental health, physical health, substance abuse, social determinants of health, and housing readiness. The Clinical Supports position is an integral part of the growing clinical team at COTS and works to support COTS service approach.
The Clinical Supports role will hold a therapy caseload of up to 20 individuals located in scattered site housing across Sonoma County (primarily based in Petaluma, Santa Rosa, and Rohnert Park) and will ensure appropriate therapeutic interventions are offered, and will assist individuals through rapport building, case conceptualization, and treatment planning. This position will also offer evidence-based, trauma-informed therapies, including psychotherapy, DBT, CBT, and somatic experiencing. These interventions will be delivered through one-on-one counseling and couples counseling, as needed.
The Clinical Supports position will also provide clinical guidance and training for non-clinical COTS staff. Weekly program meetings will be attended by the Clinical Supports position to provide guidance and offer a clinical lens to an interdisciplinary team. In the future, this position will have the opportunity to provide weekly individual and group supervision for MSW interns and Associate Social Workers collecting hours toward licensure.
In addition, in partnership with the SAFE Team, the Clinical Support Lead provides 24/7 crisis on call support to our community on a rotating basis with other COTS clinicians in support of southern Sonoma County. Calls are not frequent and include supporting SAFE Team staff via telephone; on rare occasions in-person support may be needed. This support is currently assigned weekly for at least one week out of the month.
ESSENTIAL DUTIES
- Provides individual therapy to individuals in COTS Housing and Community Referral programs.
- Utilizes trauma-informed therapeutic approaches to support individuals with healing.
- Holds the crisis line at least one week of the month.
- Collaborates alongside the case management teams to ensure client care.
- Collaborates with community partners to ensure clients receive whole-person care.
- Reviews client intake assessments, using clinical judgment, to identify and address client needs in the areas of healthcare, mental health, and substance use disorders.
- Completes treatment plans in accordance with COTS’ services.
- Coordinates with the Care Manager, the development of client’s Individualized Care Plans, based on intake assessments and client’s health goals.
- Ensures delivery of appropriate interventions and care coordination of CalAIM Enhanced Care Management (ECM) services.
- Provides clinical training for staff.
- Provides clients with self-management skills and information needed so they can manage their health and housing readiness.
- Participates in case conferencing meetings to review and evaluate the services provided to clients, and to identify opportunities to improve service delivery.
- Completes required documentation and paperwork in a timely manner, and in accordance with COTS’ and social work standards.
- Always adheres to HIPAA privacy and security requirement standards.
- Ensures HIPAA privacy and security requirements and standards are being met by COTS staff and swiftly and thoroughly address non-compliance issues as they arise.
- Ensures client files are maintained timely, organized, and meet all required standards.

QUALIFICATIONS
- Must have a valid license through the BBS in the state of California.
- Must have at least 2 years’ experience post-licensure.
- Ability to work as a member of a multidisciplinary care team.
- Experience working with vulnerable populations, ideally individuals who have or are currently experiencing homelessness.
- Experience with clinical case management is strongly preferred.
- Experience in providing individual, family, or group counseling.
- Can demonstrate a high level of knowledge regarding providing therapeutic services.
- Knowledge of and experience with case conceptualization, treatment planning, and therapeutic interventions.
- Understand the impact mental health, substance use, and trauma has on development and behavior.
- Knowledge and experience working with co-occurring populations (chronic health conditions, mental health, and substance use).
- Knowledge and experience with evidence-based practices, including trauma informed care framework, Dialectical Behavioral Therapy, Cognitive Behavioral Therapy, and/or somatic therapies.
- Fluent in trauma-informed care.
- Cultural competency and commitment to clients.
- Ethical and professional behavior.
- Bilingual (English/Spanish) highly desirable.
- Proficient in Microsoft 365.
EDUCATION
Master’s degree in social work from an accredited university required.
CERTIFICATES, LICENSES, REGISTRATIONS
LCSW license with the California Board of Behavioral Science in good standing required. Must be licensed for at least two years. Valid Driver’s License, access to reliable personally insured vehicle, and acceptable motor vehicle report required.
LANGUAGE, MATH & REASONING SKILLS
Ability to read, analyze, and interpret documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations, including medication measurements. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in written or diagram form and deal with several abstract and concrete variables.
