We are seeking a compassionate, detail-oriented, and dependable Licensed Vocational Nurse (LVN) to join our Home Health team. In this role, you will deliver high-quality, patient-centered nursing care to individuals in their homes, ensuring comfort, safety, and continuity of care. You will be responsible for assessing patient needs, implementing individualized care plans, and performing skilled nursing procedures under the direction of a Registered Nurse and physician. The LVN will collaborate closely with interdisciplinary team members, including RNs, therapists, and caregivers, to monitor patient progress, manage chronic conditions, and support recovery. This position requires strong clinical judgment, excellent communication skills, and the ability to work independently while maintaining accurate documentation and compliance with all regulatory standards.
Key Responsibilities:
- Provide direct patient care in accordance with the care plan and physician orders
- Monitor and document patient conditions, including vital signs and symptoms
- Administer medications and treatments as prescribed
- Assist with wound care, IV therapy (if applicable), and other skilled procedures
- Educate patients and families on disease management and home care techniques
