POSITION DESCRIPTION
Title: Patient Care Coordinator Pay range: $13.00- $20.00 hourly
Classification: Non Exempt/Full Time Supervisor: Operations Manager
Purpose: The Patient Care Coordinator will coordinate patient care operations for the Gabbard Health
Clinic, which provides free health care to homeless, underinsured and low income patients in the Coastal
Bend. The Patient Care Coordinator will coordinate all clinic functions such as program administration, information management, and regulatory compliance under the supervision of the Operations Manager.
Primary Duties:
With the Operations Manager assess administrative and program needs and volunteer levels.
Provide necessary monthly and quarterly reports to Operations Manager.
Assure clinic volunteers (where appropriate) are referred to medical malpractice coverage carrier.
Performs Receptionist duties for the clinic via securing patient information, maintains patient confidence by completing and safeguarding medical records, completing diagnostic coding and procedure coding and abide by all HIPAA laws and standards.
Obtain and record patient’s health history, verify patient information by interviewing patient; recording medical history; confirming purpose of visit and providing next clinic dates.
Prepares patients for examination by performing preliminary physical tests- blood pressure, weight, temperature, glucose monitoring, wound dressing application/changing and drawing any necessary lab work to be sent out for testing as instructed by physician.
Advises program participants how to interact effectively with physicians, counselors, health care facilities, and other community agencies and/or contacts as referred.
Maintains patient care database entering new information as it becomes available; verifying findings and reports.
Provides data, information, and narrative for clinic grant reports monthly, quarterly and year end.
Responsible for medical clinic budget in coordination with Operations Manager. Keeps supplies ready by inventorying stock, placing orders, verifying receipt, and log in medication donations.
Maintains medical supplies inventory and performs preventive maintenance to keep medical equipment operating.
Requirements
Four-year college degree preferred.
At least 5 years’ experience working in the medical clinic setting, i.e. Certified Medical Assistant,
LVN, or RN.
Maintain current CPR/First Aid/AED permits
Exceptional organizational and time management skills; exceptional oral and written communication skills
Ability to bend, stand, sit, and stretch for extended periods of time, climb stairs, and lift 50 pounds without assistance. Visual and hearing sufficient to comprehend written and verbal communication.
Ability to deal effectively with stress.
