Wraparound Facilitator

Job Overview
We are seeking a dynamic and compassionate Wraparound Facilitator to lead and coordinate comprehensive, individualized support plans for youth and their families. Duties

• Facilitate the development and implementation of personalized wraparound plans that address each youth’s unique needs, strengths, and goals
• Coordinate and lead team meetings with family members, service providers, schools, and community partners to ensure cohesive support strategies
• Act as a primary point of contact for families, providing ongoing guidance, encouragement, and advocacy throughout the process
• Conduct assessments to identify specific challenges related to mental health, gender issues, or other social factors impacting the youth’s well-being
• Incorporate specialized skills such as art therapy techniques or knowledge of psychopharmacology when appropriate to enhance service delivery
• Monitor progress regularly, adjusting plans proactively to maximize positive outcomes and ensure accountability
• Document all interactions thoroughly in compliance with organizational policies and maintain detailed records of case progress

Experience

• Proven experience working with youth and families in mental health or social services settings
• Strong understanding of trauma-informed care principles and family-centered approaches
• Knowledge of art therapy practices and their application in supporting emotional expression and healing
• Familiarity with psychopharmacology as it relates to adolescent mental health treatment options
• Experience addressing gender issues counseling to support diverse identities with sensitivity and respect
• Excellent communication skills with the ability to motivate and empower families from varied backgrounds
• Ability to manage multiple cases efficiently while maintaining a positive, energetic attitude
