Job description

Cornerstone Youth and Family Services is seeking Wraparound Facilitators in Shreveport, LA, for our Youth Wraparound Services program under Louisiana’s Coordinated System of Care (CSoC).


Job Summary


The Wraparound Facilitator (WF) provides family-driven, youth-guided care coordination for children and youth with complex behavioral health needs who may be involved with multiple systems (schools, child welfare, juvenile justice, behavioral health, and medical providers).


This role leads the Child and Family Team (CFT) process, develops individualized Plans of Care and Crisis/Safety Plans, coordinates services across partners, and supports families in achieving goals that improve stability and reduce the need for higher levels of care.


Essential Duties & Responsibilities


  • Engage youth and caregivers using strengths-based, culturally responsive practices that prioritize family voice and choice.
  • Complete strengths/needs discovery and assist the family in developing a clear family vision, priorities, and measurable goals.
  • Identify, invite, and support participation of natural supports and professional partners for the Child and Family Team (CFT).
  • Facilitate Wraparound meetings and ensure action steps, responsibilities, and timelines are documented and communicated.
  • Develop, implement, and update an individualized Plan of Care (POC) that includes services, supports, and outcomes.
  • Create and maintain a Crisis/Safety Plan that is practical, family-friendly, and updated when risk or circumstances change.
  • Coordinate referrals and linkages to community resources (school supports, mentoring, basic needs resources, therapy, medical care, etc.).
  • Support care transitions (e.g., hospital, crisis stabilization, residential transitions) through timely coordination and documentation.
  • Maintain frequent contact with the youth/family and team members to monitor progress and address barriers.
  • Participate in required staffing, clinical consultations, and supervision meetings.
  • Maintain timely, accurate documentation that supports Medicaid medical necessity, service delivery, and audit readiness.
  • Use secure methods for storing and transmitting protected health information (PHI) and follow all confidentiality standards.
  • Track required timelines for assessments, team meetings, plan updates, and follow-up activity per program expectations.

Louisiana Program Requirements & Expectations


  • Knowledge of Louisiana System of Care values and High-Fidelity Wraparound practice (family-driven, youth-guided, team-based, strengths-focused).
  • Participation in required trainings and maintaining ongoing credentialing/recertification as assigned.
  • Ability to work a flexible schedule and respond to urgent needs or crises as required by program structure (may include evening/weekend availability).
  • Community-based service delivery with frequent local travel to homes, schools, and partner agencies.

Minimum Qualifications


  • Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, Education, or a related field (preferred).
  • Experience in case management, child/youth services, behavioral health, foster care, school-based support, or juvenile justice (preferred).
  • Strong facilitation, de-escalation, organization, and documentation skills.
  • Valid driver’s license, reliable transportation, and current auto insurance.
  • Ability to pass required background checks and maintain confidentiality.

Knowledge, Skills & Abilities


  • Ability to build rapport quickly with youth and families and maintain professional boundaries.
  • Comfort coordinating with multiple systems and navigating complex family situations.
  • Strong written communication and ability to document clearly and professionally.
  • Basic computer skills and ability to learn electronic health record (EHR) or documentation platforms.
  • Ability to manage multiple cases, prioritize deadlines, and meet productivity expectations.


Work Environment & Physical Requirements

  • Frequent driving and community-based work in homes, schools, and partner locations.
  • Ability to sit/stand for extended periods, lift up to 25 lbs (materials/records), and work on a computer.
  • Must maintain a professional appearance and follow safety practices when in the community.
