The Facilitator at Assurance Care Services, Inc. plays a critical dual-role supporting both office operations and direct care service delivery. This position ensures continuity of care by filling in client homes when staffing gaps arise and serving as a trained Direct Support Professional (DSP) when permanent placement is secured. Facilitators also perform standard office clerk duties and are required to be on-call to respond to staffing and client needs.

Key Responsibilities
Direct Care & Field Support
- Fill in client homes as needed to ensure uninterrupted care services
- Provide direct support services in accordance with client care plans and company policies
- Serve as a trained DSP when placed in an assigned home
- Maintain accurate documentation of care provided
- Communicate client needs, incidents, or changes promptly to management
- Uphold client dignity, safety, and confidentiality at all times
Office & Administrative Duties
- Perform general office clerk duties, including but not limited to:
- Answering phones and responding to inquiries
- Filing, data entry, and record maintenance
- Assisting with scheduling and staffing coordination
- Supporting onboarding paperwork and compliance documentation
- Communicate with caregivers, clients, and supervisors regarding assignments and coverage
- Assist with maintaining employee and client records in compliance with state and agency regulations
On-Call Responsibilities
- Serve on an on-call rotation to respond to urgent staffing needs
- Coordinate or provide coverage for last-minute call-outs or emergencies
- Maintain availability during assigned on-call periods
Qualifications
- High school diploma or GED required; additional education in healthcare or human services preferred
- Previous experience in home care, direct support, or healthcare preferred
- Ability to work both in-office and in the field
- Must have reliable transportation and a valid driver’s license
- Strong communication, organizational, and time-management skills
- Ability to adapt quickly to changing schedules and environments
- Must pass background check and meet all state and agency hiring requirements
Physical & Work Requirements
- Ability to perform duties in both office and home-care settings
- Ability to stand, walk, lift, and assist clients as required by care plans
- Willingness to work flexible hours, including evenings, weekends, and on-call shifts
