Job Summary
Cornerstone Housing Partners is seeking a dedicated SASH Wellness Nurse to provide and oversee preventive wellness care and health coaching for SASH (Support and Services at Home) participants in Bennington County and Southwestern Windham County. Operating under Vermont's Nurse Practice Act and SASH Core Competencies, you will oversee participant well-being, deliver health education, and coordinate vital care services with the SASH Team and community healthcare providers to help residents safely age in place.

Duties & Responsibilities
- Care Coordination: Collaborate with the SASH team and community healthcare providers to streamline participant care.
- Preventive Care: Perform wellness assessments and implement preventive health interventions.
- Patient Education: Provide both individual and group coaching focused on self-management.
- Transitional Support: Assist participants during transitions.
- Administration: Maintain accurate, timely electronic health records and progress notes.
- Compliance: Strictly adhere to all federal HIPAA guidelines.
Requirements & Qualifications
- License: Active Registered Nurse (RN) license in the State of Vermont (Required).
- Experience: Minimum of 1 year of nursing experience (Required); 2 or more years in a clinical setting is strongly preferred.
- Certifications: Basic Life Support (BLS) certification (Preferred).
- Travel: Must possess a valid driver’s license, a dependable vehicle, and proof of active auto insurance.
- Skills: Strong organizational skills, independent judgment, and excellent verbal/written communication.

- Physical ability to lift or push up to 25 pounds.
- Emotional stability and adaptability to navigate high-pressure, changing environments.
- Capacity to interact professionally with participants experiencing difficult behavioral challenges or demanding family dynamics.
