Mission:
The Licensed Practical Nurse (LPN) (11 PM - 7:30 AM) will deliver quality nursing care to adult patients with behavioral health conditions and substance use disorders, serving as an integral member of a multidisciplinary team. The LPN will administer prescribed medications and treatments in accordance with organizational policy and clinical standards.
Essential Functions:
Administer all medications safely and in accordance with policy; document administration and monitor effectiveness.
Promptly notify the Unit Shift Manager or Director of Nursing (DON) of contracted orders and unavailable medications.
Recommend improvements to medication administration processes within the scope of clinical training.
Complete all required assessments in a timely and accurate manner.
Ensure that ongoing patient assessments are reflected in treatment plans and progress notes.
Implement special precautions as indicated, in accordance with established policies.
Provide reassurance and support to patients and families during hospitalization and treatment.
Maintain vigilance and promptly identify potential patient care concerns.
Participate actively in treatment planning by sharing relevant observations and clinical information.
Assist patients with treatments and identify when additional help is needed with activities of daily living (ADLs).
Demonstrate respect for patient rights and provide individualized care.
Follow proper infection control procedures; articulate and demonstrate knowledge of infection prevention protocols.
Direct staff to ensure patients are engaged in activities and that schedules are followed.
Conduct community meetings and lead groups as needed.
Monitor task assignments to ensure completion.
Identify and promptly report or correct unit issues or patient care concerns.
Communicate effectively with the Unit Shift Manager to resolve unit and staffing issues.
Ensure timely faxing of admissions, transfers, and discharges to the Admissions Office.
Verify that unit rounds and interval checks are conducted and documented accurately.
Treat all patients with dignity and respect.
Maintain confidentiality of patient and facility information; adhere to release-of-information policies.
Utilize effective verbal and non-verbal communication techniques to promote positive patient outcomes.
Job Requirements:
Valid Licensed Practical Nurse (LPN) license in the Commonwealth of Pennsylvania.
At least two (2) years of experience in substance use or mental health nursing within a community healthcare setting.
Current CPR certification.

Ability to exercise self-control in potentially volatile situations, including verbal or physical confrontations.
Capacity to focus and work effectively amid distractions such as noise, conversations, and foot traffic.
Ability to manage frequent interruptions and transition between tasks seamlessly.
Flexibility and patience when dealing with differing opinions or challenging circumstances.
Physical ability to stoop, kneel, crouch, crawl, reach, stand, walk, push, pull, lift, grasp, and use touch to identify the attributes of objects (e.g., size, shape, temperature, texture).
Ability to express and exchange ideas verbally, conveying details or instructions clearly. Must be able to perceive sounds with no greater than a 40 dB loss at 500 Hz, 1000 Hz, or 2000 Hz, with or without correction.
Ability to process detailed information and make sound decisions based on verbal communication.
Must be able to exert up to 100 pounds of force occasionally to lift, carry, push, pull, or move objects.
Visual acuity (with or without correction) sufficient to perform administrative tasks and monitor patients during unit activities.
While occasional exposure to temperature changes may occur, the work is primarily performed indoors with minimal exposure to adverse environmental conditions.
May be required to travel throughout the city, including areas where drug trafficking activity is prevalent.
