Responsibilities
● Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
● Provide skilled physical therapy services in patients’ homes.
● Help patients improve mobility, strength, balance, and functional independence.
● Monitor patient progress and adjust treatment plans as needed.
● Educate patients and caregivers on therapeutic exercises, safety, and fall prevention.
● Coordinate care with physicians, nurses, and other healthcare professionals.
● Complete accurate and timely clinical documentation.
● Follow agency policies and applicable state and federal regulations.
Qualifications
● Active Florida Physical Therapist (PT) license.
● Previous home health experience preferred but not required.
● Strong communication, clinical, and organizational skills.
● Ability to work independently and manage assigned patients.
● Reliable transportation and willingness to travel within Broward County.
● Current professional liability insurance and other required credentials, as applicable.
