Job Overview
Copper Falls Hospice is looking for an experienced LMSW to join our hospice family. The applicant we are looking for needs to have a passion for hospice and for providing exceptional service. The position we are looking to fill is PRN/PT with the option to grow into FT if desired.
Responsibilities
- Performs and documents initial psychosocial, emotional, bereavement assessments and assists in the development and implementation of goal-directed plan of care.
- Conducts ongoing reassessments of patient/family needs and counseling, as required.
- Provides short term crisis intervention and individual or family counseling, when indicated.
- Provides services, under the direction of a physician who approves the plan of care.
- Participates as a member of the Bereavement Team, as assigned.
- Observes, assesses and brings to IDG conferences information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
Requirements
- A degree in Social Work is required.
- Understanding of Medicare processes and resources available for clients.
- Excellent communication skills, both written and verbal, with the ability to engage effectively with diverse populations.
- Ability to work collaboratively within a team environment while also being self-motivated.
- Strong organizational skills with attention to detail for documentation purposes
