Position Overview

Lighthouse Hospice is growing and seeking a compassionate and dedicated Medical Social Worker (PRN) to join our interdisciplinary care team based out of Eagle, Idaho. In this vital role, you will provide emotional, psychosocial, and practical support to patients and their families as they navigate the challenges of serious illness and end-of-life care.

As a Medical Social Worker, you will help ensure that each patient’s journey is met with dignity, respect, and understanding, offering support that honors individual values, cultures, and beliefs. Your clinical expertise and compassionate presence will be key in helping patients and families find comfort, clarity, and peace throughout the hospice experience.

Key Responsibilities

  • Psychosocial Assessments: Conduct comprehensive assessments to identify emotional, social, and financial needs of patients and families.
  • Care Planning: Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Crisis Intervention: Provide counseling and support during times of crisis, including end-of-life issues.
  • Grief and Bereavement Support: Offer grief counseling and facilitate bereavement services for to up to a year and a half following the patient's death, surpassing Medicare requirements for hospice services.
  • Community Resources: Assist patients and families in accessing community resources and services.
  • Documentation: Maintain accurate and timely records of social work services provided, ensuring compliance with hospice documentation standards.

Qualifications

  • Education: Master's degree in Social Work (MSW), LCSW or LMSW from an accredited institution.
  • Licensure: Current licensure as a Licensed Social Worker (LSW) in the state of Idaho.
  • Experience: At least one year of experience in social work, preferably in a healthcare or hospice setting.
  • Skills: Strong interpersonal and communication skills, with the ability to work effectively in an interdisciplinary team.
  • Ethical Standards: Adherence to the NASW Code of Ethics and the NASW Standards for End of Life Care.

Additional Preferred Qualifications

  • Certification: Certified Hospice and Palliative Social Worker (CHP-SW) certification is preferred.
  • Language Skills: Proficiency in Spanish or other languages is a plus
