- Assess physical and psychosocial needs, adapting care plans accordingly.
- Educate patients and families, establishing realistic goals and treatment plans.
- Coordinate care and discharge planning with multidisciplinary teams.
- Complete physician orders, medication reconciliation, and documentation per policies.
- Manage caseloads, prioritize visits, and triage communications promptly.
- Provide emotional support and manage patient/family stress effectively.
- Maintain knowledge of payer regulations, documentation standards, and safety protocols.
- Valid RN licensure in the respective state.
- Minimum of one year of acute medical/surgical nursing experience.
- CPR certification and a valid driver’s license.
