Responsibilities: Manage patient census, close care gaps, coordinate community resources, provide education on chronic condition management, perform medication reconciliation, ensure timely service delivery, resolve patient inquiries, and manage referrals and appointments.
Qualifications & Skills: Active multi-state or compact LPN license, technical proficiency with Microsoft Office, excellent communication skills, and experience in patient-facing roles. Bilingual skills and Epic experience are advantages.
Requirements: High-speed Wi-Fi, HIPAA-compliant home workspace, and ability to sit for extended periods while communicating effectively.
