Key Responsibilities:
- Conducting client assessments and reassessments as assigned
- Performing supervisory visits with clients
- Ensuring compliance with agency policies, state regulations, and care plans
- Communicating effectively with clients, families, and office staff
- Providing documentation and reports in a timely manner
Requirements:
- Active Licensed Practical Nurse (LPN) license in good standing
- Experience in home care or community-based services preferred
- Strong communication and organizational skills
- Reliable transportation and ability to travel to client homes
- Ability to work independently and professionally
