Key Responsibilities:
Deliver in-home nursing care per individualized care plans
Complete comprehensive OASIS assessments and documentation
Administer medications, monitor vital signs, and manage wound care and chronic conditions
Provide patient and family education on treatments and disease management
Coordinate care with interdisciplinary team to ensure continuity
Use WellSky/Kinnser EMR for accurate documentation
Travel within designated areas and receive mileage reimbursement
Qualifications:
Active New Mexico RN license or valid Compact RN license
Current AHA BLS certification
Minimum 1 year of recent home health nursing experience
OASIS documentation experience required
Valid driver's license, reliable vehicle, and auto insurance required
Preferred Skills:
Experience with WellSky/Kinnser EMR
