  • Ensure compliance with HIPAA and all applicable federal, state, and local regulations.
  • Complete and oversee pre-admission, admission, quarterly, and ongoing resident assessments.
  • Develop, implement, and update individualized care plans in collaboration with residents, providers, and the interdisciplinary team (IDT).
  • Monitor resident conditions and adjust interventions and care plans as appropriate.
  • Oversee medication systems, including receipt, documentation, administration, and discontinuation practices.
  • Provide clinical guidance related to acute care, hospice coordination, and home health services as applicable.
  • Maintain accurate, timely, and compliant clinical documentation and resident records.
  • Collaborate with physicians, pharmacists, case managers, and external healthcare professionals.
  • Lead quality assurance and performance improvement initiatives.
  • Ensure staff understanding and adherence to facility policies, procedures, and clinical standards.
  • Schedule and supervise nursing staff with support from designated leadership personnel.
  • Serve as the primary clinical resource for nursing-related concerns and provide hands-on support when needed.
  • Perform additional duties as assigned to support resident care and operational needs.

Minimum Qualifications

  • Current, active, and unencumbered LPN or RN license in the State of Washington.
  • Nursing experience in assisted living, long-term care, memory care, hospice, home health, or acute care settings.
  • Experience supporting residents with dementia and behavioral health needs preferred.
  • Demonstrated leadership, supervisory, and team development experience.
  • Strong knowledge of HIPAA and resident confidentiality standards.
  • Excellent organizational, time management, and prioritization skills in a fast-paced environment.
  • Sound clinical judgment and ability to manage complex resident care situations.
  • Strong verbal and written communication skills, including documentation review and interdisciplinary collaboration.
  • Proficiency with electronic health records (EHR) and healthcare documentation systems.
  • Experience with quality improvement processes and staff training initiatives.
  • Ability to work independently while collaborating closely with the Administrator and leadership team.
  • Successful completion of a criminal background check.
  • Certifications in nursing leadership or administration preferred.
