Minimum Requirements:
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Washington State PT license required to start.
- BLS certification from AHA or ARC may be required.
- Experience providing care in home health settings preferred.
- Experience supervising PTAs and coordinating with home health aides preferred.
- Familiarity with OASIS assessments, ICD-10 coding, and documentation for home health required.
- Ability to manage a typical caseload of approximately 25 visits per week (20 visits/week when paired with a PTA).

Responsibilities:
- Provide treatments according to physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
- Work with home care personnel in patient care planning.
- Direct and supervise Physical Therapy Assistants and Home Health Aides, as assigned.
- Take initial patient history and perform initial evaluations.
- Consult with physicians regarding changes in treatment.
- Instruct patients and families in home programs and activities of daily living.
- Participate in Quality Assessment and Performance Improvement activities as assigned.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and documentation for significant changes in condition.
- Appropriately utilize ICD-10 codes in documentation.
