Director of Rehabilitation – Physical Therapist
Location: Full-Time | Leadership Role

A respected skilled nursing and rehabilitation facility in the Portland, OR metro area is seeking an experienced Director of Rehabilitation to oversee and grow its therapy department. This leadership role is ideal for a licensed PT or SLP with proven success in clinical supervision, program development, and team performance management in a long-term care setting.
As the Director of Rehabilitation, you will lead all therapy operations, manage a multidisciplinary therapy team, and develop systems that ensure quality outcomes, operational excellence, and regulatory compliance. You’ll drive program growth, promote a culture of clinical excellence, and build strong partnerships with referring providers and internal clinical teams.
Key Responsibilities
- Lead and manage therapy staff, including PTs, OTs, SLPs, COTAs, and Rehab Aides.
- Build and maintain a supportive and collaborative team environment.
- Ensure therapy services meet all state, federal, and payer compliance standards.
- Develop special clinical programming to strengthen existing therapy services (e.g., dementia care, restorative programming, fitness).
- Drive census and revenue growth through caseload development and community outreach.
- Oversee therapist scheduling, patient assessments, treatment planning, and clinical documentation.
- Participate in interdisciplinary care planning, clinical rounds, and discharge planning.
- Conduct internal audits and performance reviews to ensure excellence in service delivery.
- Maintain up-to-date knowledge of Medicare, Medicaid, and private insurance reimbursement guidelines.
- Represent the department in facility leadership meetings and QAPI initiatives.
Qualifications
- Bachelor’s, Master’s, or Doctorate in Physical Therapy or Speech Language Pathology.
- Active PT or SLP license in the state of Oregon.
- CPR certification and valid driver’s license.
- MDS (RAC-CT) or other advanced certification preferred.
- Minimum 3 years of rehabilitation experience; 6 months in a skilled nursing or long-term care setting preferred.
- Proven leadership, staff supervision, and program management experience.
- Strong knowledge of rehab-related billing, documentation, and regulatory requirements.
- Ability to foster a high-performing, resident-centered therapy culture.
