Director of Nursing (DON) – RN

Reliant Living Centers of Oklahoma, Inc.
Oklahoma City / Del City, OK
Full-Time | Leadership Position

About Reliant Living Centers

Reliant Living Centers of Oklahoma, Inc. is a nonprofit organization providing residential and habilitative services to individuals with intellectual and developmental disabilities in an ICF/IID setting.

Our mission is centered on providing exceptional care while promoting dignity, independence, safety, and quality of life for the individuals we serve.

Never Done. Never Outdone.

We are seeking an experienced and dependable Registered Nurse (RN) to serve as our Director of Nursing (DON) and lead nursing services across our ICF/IID communities.

This is a hands-on leadership position for an RN who understands that excellent nursing leadership requires more than managing paperwork. The DON must be visible, organized, responsive, clinically knowledgeable, and committed to supporting both residents and staff.

Position Summary

The Director of Nursing is responsible for the overall leadership, coordination, and oversight of nursing services.

The DON works closely with the Administrator, nursing staff, QIDP, case management, physicians, advanced practice providers, pharmacy consultants, direct-care staff, residents, guardians, and other members of the interdisciplinary team to ensure residents receive safe, timely, person-centered healthcare.

The successful candidate will establish clear expectations, develop strong nursing systems, monitor clinical follow-through, support staff education, and maintain continuous readiness for state and federal surveys.

Key Responsibilities

  • Provide leadership and supervision for the nursing department, including LPNs, CMAs, and other nursing-related personnel.
  • Ensure residents receive nursing services appropriate to their individual medical and healthcare needs.
  • Monitor changes in resident condition and ensure timely assessment, intervention, physician/provider notification, follow-up, and documentation.
  • Oversee physician orders, medication administration systems, MARs, treatments, laboratory work, diagnostic testing, appointments, and clinical follow-up.
  • Ensure new and changed medication orders are accurately implemented and communicated.
  • Maintain strong medication-management practices and work collaboratively with the consultant pharmacist and healthcare providers.
  • Participate in the interdisciplinary team and provide appropriate nursing input into Individual Program Plans (IPPs), medical plans, and resident care planning.
  • Ensure required nursing assessments, health reviews, physical examinations, screenings, immunizations, laboratory testing, and other preventative healthcare services are completed and documented.
  • Monitor hospitalizations, emergency room visits, changes in condition, and resident readmissions to ensure continuity of care.
  • Review incidents involving medical or nursing concerns and ensure appropriate assessment, notification, investigation, documentation, and corrective action.
  • Assist the Administrator and interdisciplinary team with reportable incidents and regulatory follow-up when appropriate.
  • Maintain and oversee infection prevention and control practices.
  • Educate nursing and direct-care staff regarding infection control, signs and symptoms of illness, basic health needs, emergency response, and resident-specific medical concerns.
  • Ensure nursing personnel work within their appropriate scope of practice and that delegation to Certified Medication Aides is performed appropriately.
  • Maintain nursing competency standards and identify additional education or corrective training when needed.
  • Provide orientation and ongoing training for nursing personnel.
  • Conduct routine nursing audits, including medication records, physician orders, treatments, assessments, appointments, laboratory results, documentation, and follow-up.
  • Identify clinical trends, recurring documentation problems, medication concerns, and potential resident-safety issues before they become larger problems.
  • Participate in quality assurance, performance improvement, risk management, and corrective-action activities.
  • Maintain compliance with applicable CMS ICF/IID Conditions of Participation, Oklahoma requirements, professional nursing standards, and Reliant policies.
  • Work collaboratively with the Administrator to prepare for state and federal surveys and respond to identified deficiencies.
  • Assist with development and implementation of plans of correction when nursing-related deficiencies are identified.
  • Help ensure adequate nursing coverage and participate in an appropriate on-call/after-hours clinical support system.
  • Foster a professional nursing department built on accountability, communication, teamwork, resident dignity, and timely follow-through.

Required Qualifications

  • Current, active and unencumbered Oklahoma Registered Nurse (RN) license.
  • Strong clinical assessment and nursing judgment.
  • Demonstrated ability to lead and supervise nursing personnel.
  • Strong organizational, documentation, and follow-through skills.
  • Ability to communicate effectively with physicians, APRNs, pharmacists, residents, guardians, direct-care staff, management, and regulatory personnel.
  • Ability to prioritize multiple clinical and administrative responsibilities.
  • Working knowledge of medication administration, physician orders, infection prevention, resident assessment, and nursing documentation.
  • Ability to participate in an on-call leadership rotation and respond appropriately to urgent clinical concerns.

Preferred Qualifications

  • Previous Director of Nursing, Assistant Director of Nursing, Nurse Manager, or comparable nursing leadership experience.
  • Experience in an ICF/IID, IDD, developmental disabilities, long-term care, residential healthcare, or similar setting.
  • Familiarity with CMS survey requirements and regulatory compliance.
  • Experience with medication-management systems, pharmacy reviews, incident investigations, quality assurance, and staff education.
  • Experience working with an interdisciplinary team.
  • Experience with electronic health records, including PointClickCare or similar systems.

ICF/IID experience is highly desirable but not required for an experienced nursing leader who is willing to learn the ICF/IID model and regulatory requirements.

What We Are Looking For

We are looking for a nursing leader who:

  • Is present and accessible to staff.
  • Addresses problems rather than allowing them to continue.
  • Communicates clearly and professionally.
  • Treats residents and employees with dignity and respect.
  • Understands the importance of accurate documentation and timely follow-through.
  • Can coach and educate staff while still holding them accountable.
  • Works collaboratively with administration rather than operating nursing as a separate department.
  • Remains calm and uses sound nursing judgment when difficult situations arise.
  • Takes ownership of the nursing department and continuously looks for ways to improve resident care.

Schedule

Full-Time

Primarily Monday through Friday with administrative and clinical availability as necessary. The Director of Nursing must be available for occasional after-hours consultation, emergencies, survey activity, and other situations requiring nursing leadership.
