Duties
- Provide clinical leadership and supervision to hospice nursing staff, fostering a collaborative and motivated team environment
- Develop, review, and update individualized care plans aligned with patient needs, preferences, and regulatory guidelines
- Ensure compliance with all federal, state, and local healthcare regulations including Medicare and HIPAA standards
- Oversee documentation review, medical records management, and accurate record-keeping within EMR/EHR systems
- Monitor patient care quality through regular assessments, audits, and performance evaluations to uphold excellence in hospice services
- Manage shift assignments, resource allocation, and staffing schedules to ensure optimal coverage and patient safety
- Facilitate staff training on topics such as resident's rights, long-term care regulations, and documentation best practices
- Collaborate with interdisciplinary teams to coordinate comprehensive care that addresses physiology, anatomy, memory care, and end-of-life needs
- Lead efforts in regulatory compliance audits and implement corrective actions as needed to meet accreditation standards
- Promote a culture of continuous improvement by reviewing clinical procedures and implementing best practices in hospice nursing
Requirements
- Valid RN license with current registration in the state of practice
- Proven experience in hospice care or home health agencies with demonstrated leadership capabilities
- Strong knowledge of long-term care facility operations, assisted living, nursing home environments, and acute care settings
- Familiarity with MDS (Minimum Data Set), Medicare regulations, and long-term care regulations governing hospice services
- Extensive experience supervising nursing teams within home health or hospice settings; management or nurse management experience preferred
- Proficiency in EMR/EHR systems for documentation review and medical records management
- Deep understanding of patient care principles related to physiology, anatomy, memory care, and end-of-life processes
- Knowledge of HIPAA compliance standards for safeguarding patient information
- Excellent communication skills to effectively lead staff meetings, coordinate interdisciplinary teams, and engage with patients’ families
- Ability to manage multiple priorities efficiently while maintaining attention to detail in documentation review and shift management
