Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking Licensed Practical Nurses (LPNs) to provide comprehensive end-of-life care and support to our patients and their families in their homes in the Beaver County Area. As a Hospice LPN, you will be an integral part of the interdisciplinary team that collaborates with our physicians, social workers, spiritual and volunteer coordinators to ensure the comfort and dignity of our patients in their final stages of life. We have both full and part time Hospice LPN positions available. Schedule includes working one weekend a month and an on-call rotation. QUALIFICATIONS for HOSPICE LPN: Current LPN license in the state of Pennsylvania Demonstrates excellent clinical skills Solid computer skills, including Word Excellent organizational skills with attention to detail Outstanding communication skills (both written and verbal) Reliable independent means of transportation with a valid Pennsylvania driver's license RESPONSIBILITIES FOR THE LICENSED PRACTICAL NURSE POSITION: Assess and monitor the physical, emotional, and spiritual needs of our hospice patients and create individualized care plans. Educate patients and their families about the disease process, care options, and end-of-life decisions. Provide pain management and symptom control measures to alleviate discomfort and improve quality of life. Advocate for our patients, ensuring their wishes and preferences are respected and honored.
