The Oasis Workflow Registered Nurse (RN) is responsible for overseeing quality measures and data to ensure that submitted information aligns with OASIS guidance and regulatory requirements.
- Conduct reviews of OASIS assessments in accordance with best practice guidelines.
- Evaluate quality and process measure outcomes using advanced data mining software.
- Improve ancillary services by analyzing assessments to foster positive patient outcomes.
- Detect inconsistencies and errors in clinical documentation, reporting findings to the Quality/Education teams.
- Take part in Quarterly Quality Record Reviews.
- Work alongside the education team to provide quarterly OASIS training.
- Participate in quality and rehospitalization meetings, assessing clinician scorecards and formulating corrective action plans as needed.
- Perform audits to ensure adherence to all regulations governing home health and the appropriate use of services.
- Current Pennsylvania RN license.
- Minimum of two (2) years of home health experience with progressive responsibilities.
- OASIS certification is recommended; if not currently certified, must obtain within one year.
- Knowledge of agency policies, federal and state regulations.
- Strong interpersonal, communication, and organizational skills with evidence of leadership activities.
- Proficient in home health computer documentation systems.
- Reliable transportation with a valid driver’s license and insurance as required by state law.
