POSITION OVERVIEW: Program Outreach and Retention Specialist
The Program Outreach and Retention Specialist is tasked with outreaching and engaging participants throughout the duration of their pre and post release programming as well as after graduation. This role will check in with the participant, answer any questions, assess barriers, connect participants with care team members, data collection, and follow up on participants’ achievements post programming. The Program Outreach and Retention Specialist will be the bridge for participants after their release before they arrive at the Center. They will focus on maintaining strong alumni relationships to ensure participants long term success.
KEY RESPONSIBILITIES - YOUR ROLE WILL INVOLVE:
- Establish and maintain supportive relationships with participants throughout the entirety of the program and after graduation.
- Conduct outreach calls to participant’s approved family members prior to participant’s release to start relationship building
- Outreach participants to set up Landing intake immediately upon release
- Form meaningful, sustainable relationships with participants to support with reentry needs
- Increase participant program retention
- Identify and address barriers to continued program participation
- Provide crisis intervention as needed
- Obtain critical grant data during outreach calls
- Provide regular updates and reports to the executive team
- Ensure accurate data entry and tracking in the Concordance participant database system
- Utilize incentives program for participants to support data collection
- Facilitate focus groups post-programming
- Establish a communications channel that encourages participants, their loved ones, and alumni to contact Concordance when needing additional support including case management needs; therapy services; employment, and training services
- Collaborate with participants during the various phases to establish trust and rapport

PERSONAL ATTRIBUTES:
- Desire and ability to build relationships and engage with participants and their loved ones
- Collaborative attitude and excitement about working in a multi-disciplinary environment
- Able to resolve conflict in crisis situations, problem solve, and provide triage support to participants over the phone
- Excellent verbal and written communication skills
- Experience in tracking participant outcomes
- Availability to travel locally as well as working some evenings and weekends to connect with participants at times when they are available
- Ability to organize, prioritize and handle multiple work assignments
- Familiarity with common office equipment
- Passion for our mission
REQUIRED EDUCATION, AND LICENSURE:
- Bachelor’s degree or 3 years of relevant work experience
- Proven track record of success in relationship building
- Superior interpersonal skills
- Experience with working with justice-involved adults or other vulnerable populations (preferred).
- Lived experience (preferred)
SKILLS AND COMPETENCIES:
- 1-3 years of experience in relationship building
