What are the responsibilities and job description for the CUA Well-Being Specialist position at CONCILIO DE ORGANIZACIONES HISPANAS?
General Description:
This position is key to ensuring medical needs of children are met. The role will act as a support to CM team in obtaining and reviewing vaccination records, medical/dental/specialist records; acts as a liaison with the DHS Health Management Unit and ensures all children in placement have medical coverage; supports the Prevention team with community efforts, emergency coverage, and other responsibilities as assigned.
Duties and Responsibilities:
• Ensure that children and youth have medical insurance.
• Review new case lists for children in care who will need insurance activation, birth certificates, vaccination/medical records, or other related supports
• Review internal and external audits to identify children who are missing vaccination/medical records, birth certificates, BHI or other related documents.
• Act as a liaison between CUA Case Management and DHS Health Management Unit • Ensure tracking of all Wellbeing requests as well as timely follow up with both DHS and CUA CM
• Participate in multidisciplinary meetings or conferences as appropriate • Be familiar with and follow DHS CUA Practice Guidelines as well as any published changes and additions to same.
• Utilize Strengthening Families framework in the performance of duties. • Maintain high degree of efficiency and organization, responding accurately and professionally to high volume of emails and requests
• Maintain confidentiality and ensure HIPPA compliance. Ensure that information concerning families is not accessed or disclosed except in the performance of duties and as allowed by law.
• Immediately report to supervisor any concerns about the safety of children or youth in the household or placement facility.
• Complete all paperwork and other documentation timely, accurately, and in accordance with the DHS CUA Practice Guidelines.
• Maintain compliance with all applicable laws, regulations, and bulletins issued by the Department of Public Welfare (DPW) Office of Children Youth and Families. Abide by the Social Work Code of Ethics.
• Able to work a flexible schedule to meet the needs of children, youth, and families and to cover emergencies.
• Consult with DHS Nurses, when necessary.
• Consult with the DHS Medical Eligibility Unit (MEU) and the DHS Health Management Unit (HMU) as needed to ensure continuity of medical insurance and care.
• Other duties as assigned.

Qualifications:Qualifications
• A high school diploma or equivalent is required; an associate degree or higher is preferred; an equivalent of education and experience is to be considered.
• Five years of operational experience or facilities management experience required
• Valid Driver’s License
• Flexibility to travel to and from Concilio properties during, before, and after shifts.
• On-Call availability/ Ability to work onsite and remotely, as needed
• Must have a valid driver's license and insurance, be able to travel, and be flexible.
• Facility certification and OSHA certifications Required

Physical Demands

Work Environment
While performing the duties of this job, the employee is frequently exposed to moving mechanical parts. The employee is occasionally exposed to wet and humid conditions, high, precarious places, fumes or airborne particles, toxic or caustic chemicals, outside weather conditions, the risk of electrical shock, and vibration. The noise level in the work environment is usually moderate to occasionally loud.
