Responsibilities

• Deliver high-quality nursing care tailored to the needs of elderly residents, including medication administration, vital signs monitoring, and physical assessments
• Collaborate with interdisciplinary teams to develop and update individualized care plans that promote residents’ health and well-being
• Perform specialized procedures such as IV infusion, catheterization, wound care, and tube feeding with precision and adherence to safety protocols
• Conduct triage assessments to prioritize urgent needs and coordinate appropriate interventions promptly
• Maintain accurate and detailed medical documentation using electronic health record (EHR) systems while ensuring compliance with HIPAA and long-term care regulations
• Monitor residents for signs of acute pain or infectious disease, implementing appropriate interventions or escalating as necessary
• Educate residents and their families about health conditions, medication management, and self-care strategies to support ongoing wellness

Qualifications

• Valid licensure as an LPN or RN with current state certification and licensing in good standing
• Demonstrated experience working with geriatric patients in settings such as skilled nursing facilities, assisted living, or long-term care environments
• Proficiency in EMR/EHR systems like Epic, Cerner, or Athenahealth for efficient medical documentation
• Strong knowledge of medical terminology, anatomy, physiology, and long-term care regulations
• Experience with complex procedures including IV insertion, venipuncture, sterile technique, and medication administration
• Familiarity with dementia care, Alzheimer’s disease management, hospice care principles, and memory care practices
• Ability to perform physical examinations, vital signs assessment, and patient evaluation accurately
• Excellent caregiver communication skills with a focus on patient-centered care and teamwork
