Full-Time RN Case Manager – Hospice Nurse (Austin Area)

What You’ll Do:
- Manage a caseload of hospice patients in the Austin area.
- Provide skilled, compassionate nursing care and coordinate individualized care plans.
- Complete routine visits, admissions, recerts, and IDT documentation.
- Collaborate with the interdisciplinary team and participate in biweekly IDT meetings.
- Communicate effectively with patients, families, and providers.
- Maintain accurate, same-day charting with a provided tablet. Deliver quality end-of-life care that reflects our values of compassion and respect.
What We’re Looking For:
- Current RN license in Texas.
- Hospice or home health experience preferred (training available for the right candidate).
- Excellent communication, organization, and teamwork skills.
- Reliable transportation and willingness to travel locally within the service area. A passion for caring for people and making a meaningful impact every day.
