You'll work with patients recovering from surgery, illness, injury, and hospitalization while helping them safely improve mobility, balance, endurance, strength, and functional independence.

About the Role
As a Physical Therapist Assistant (PTA), you'll provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist in a Skilled Nursing Facility (SNF). You'll work with patients recovering from surgery, illness, injury, and hospitalization while helping them safely improve mobility, balance, endurance, strength, and functional independence.
Responsibilities
- Deliver physical therapy treatments established by the supervising Physical Therapist.
- Monitor patient response to treatment and communicate progress with the Physical Therapist.
- Assist patients in improving mobility, balance, gait, strength, endurance, and functional independence.
- Accurately document treatments, patient progress, and daily billing.
- Collaborate with interdisciplinary team members to provide coordinated patient care.
- Educate patients, families, and caregivers on home exercise and maintenance programs.
- Participate in care planning and discharge planning as appropriate.
- Maintain compliance with all state licensure requirements and Concept Rehab policies.
- Demonstrate Concept Rehab's core values of Commitment, Respect, and Integrity.
- Associate Degree from an accredited Physical Therapist Assistant program.
- Current Physical Therapist Assistant (PTA) license in the state of practice (or eligible for licensure).
