Position Overview:

This position is under direct supervision of the Director of Nursing and administrative supervision of the Regional/Service Director for ICF/MR. Is responsible for assisting the Director of Nursing and clinical staff in delivering services to the individuals. Is responsible for the safety and well being of all individuals living in the facilities

 

Essential Duties & Responsibilities:

 

Administrative:

  • Prepare and submit required documentation for reimbursement of work-related mileage, use of PTO, timesheets, and any other required reports accurately and on a timely basis.
  • Obtain approval and submit invoices for accounts payable.
  • Maintain and utilize the home and other company property entrusted to your care. Monitor the use of resources such as, but not limited to, food, supplies, clothing, small equipment items and other appropriate needs for the program/individual.
  • Obtain petty cash as needed and follow required procedures related to expenditure and receipts.
    • Participate in mandatory house meetings as assigned by supervisor.

Regulatory:

  • Monitor compliance with all licensure and regulatory standards and follow-up on all programmatic recommendations and plans of correction. Keep supervisor and/or ICF/MR Director informed of follow up.
  • Implement ComServ, Inc. policies and procedures.
  • Monitor food supplies, cleaning supplies, work orders, etc in order to maintain and provide a clean, secure home environment for the individuals.
  • Have a working knowledge and adheres to all ComServ, Inc. Policies and Procedures.

 

Documentation:

  • Document program specific information per rules and regulations of the service provided in the residence.
  • Document all necessary information from OT/PT appointments.
  • Document all medical in-service trainings conducted for staff.
  • Document medical changes in the service record as required.
  • Follow-up and document the required nursing section on all incident reports as required.
  • Document medication changes on the Medication Administration Record.
  • Document any needed changes to Physician Orders per physician request and signature.

 

Clinical:

    • Act as a member of the Interdisciplinary Team as assigned.
    • Participate in Interdisciplinary Team meetings as assigned by supervisor.
    • Act as liaison for staff and clinical team members.
    • Monitor facilities periodically (AM, PM and weekends) while individuals are present.
    • Monitor Medication Administration Records for correctness and accuracy.
    • Ensure that individuals in the facilities have opportunity to integrate into the community, to decorate their bedrooms, shopping for personal items and make other personal choices.
    • Ensure that staff training for medical needs is provided in a timely and economical fashion.
    • Communicate with all team members concerning medical needs, hygiene, health related issues, human rights approvals and any other issues regarding individuals.
    • Report human rights violations to your supervisor/Administrator on call.
    • Administer and/or monitor medication administration and treatment as needed.
    • Ensure that all prescribed treatments, orders and medication instructions are properly followed.
  • Follow up with any medical recommendations in a timely fashion and keep supervisor informed.
  • Attend all Clinical Team meetings.
  • Maintain formal and informal contacts with the legally responsible person as needed.
  • Coordinate and assist with all medical appointments.

 

 

Direct Support:

  • Provide direct support as needed or when deemed necessary by supervisor and/or ICF/MR Director.
  • Provide acute medical care as needed for the individuals living in facilities.

 

Other:

  • Be flexible with your work schedule.
  • Participate in the nursing on call rotation for all facilities.
  • Participates on committees as assigned by supervisor.
  • Ensure and support cultural competency in conducting responsibilities that is reflective of the community and individuals served related to culture, age, gender, sexual orientation, spiritual beliefs, socioeconomic status, and language.
  • Adhere to ComServ, Inc. mission, vision and philosophy and Code of Conduct.
  • Distribute medical supplies for each facility as needed.
  • Attend all organizational training required for position. Attends all outside training and education as approved or assigned by your supervisor and/or ICF/MR Director.
  • Participate in company functions as directed by your supervisor and/or ICF/MR Director.
  • Act as a role model for other ComServ, Inc. staff and individuals receiving services.
  • Ensure that all ComServ, Inc. equipment or vehicle used in conducting the responsibilities of this position is used in a responsible manner and maintained appropriately.
  • Perform any other duties as deemed appropriate and approved by your supervisor and or ICF/MR Director.
Qualifications:

Minimum Education & Experience Requirements:

Graduation from Licensed Practical Nurse Program.

 

Knowledge, Skills & Abilities Required:

Ability to work well with others, communicate clearly, verbally and in writing, possess leadership/supervisory and management ability. Ability to train and supervise staff. Ability to coordinate and instruct staff. Ability to establish rapport and relate to the individuals served, their families, community agencies, and groups, and a variety of other persons they might have contact with related to service delivery. A valid drivers license in the state of residence and a safe driving record acceptable under ComServs liability carrier is required.

 

Professional Licensure Requirements: Valid NC Licensed Practical Nurse (LPN). Complete and provide documentation of 30 hours of continuing education every two years.

 

Physical Requirements:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Must be able to operate a variety of automated office equipment including a computer to communicate via e-mail, copier, fax machine, wheelchairs, lifts where applicable, and other resident related equipment. Must be able to exert up to seventy-five pounds of force occasionally and/or frequently to lift, carry, push, pull, or otherwise move objects. Physical demands are in excess of sedentary work; position requires walking, standing, reaching, stooping, etc. long periods of time. Must be able to lift and/or carry weights of up to one hundred fifty pounds with assistance.
