What are we looking for in a Care Navigator?
Care Navigators work within our Hub & Spoke team to help clients and providers coordinate substance use treatment with other community services. The Hub & Spoke model is a network of community providers that work together to provide integrated care for individuals receiving medicated assisted treatment for opioid use disorders. This integrated approach assists clients with multiple facets of their recovery, regardless of which service provider forms the initial connection. The Care Navigators provide case management for clients to assist with accessing primary care, employment services, transportation, housing, and other resources. The schedule for this role is Monday-Friday day shift.
Care Navigator duties may include:
- collaborating with the client to develop a comprehensive treatment plan that includes each of the services being utilized
- coordinating treatment & supportive services between providers within the Hub & Spoke network to most effectively serve clients
- assessing clients for safety and risk to self or others, and taking appropriate crisis intervention measures when indicated
- monitoring clients’ progress with symptoms and response to medications
- advocating for clients to attain services and benefits to further recovery goals
- thoroughly completing all documentation in a timely, client-centered manner
Qualifications:
- Bachelor's degree in social work, psychology, or related field
- Must be eligible for registration as Agency Affiliated Counselor
- Must have valid driver's license and vehicle liability insurance
