Our Patient Care Coordinator & Pharmacy Technicians are helping pioneer a new, patient-centered model of care that goes beyond traditional dispensing to improve the health and well-being of the patients we serve.

This unique hybrid role combines approximately 50% traditional pharmacy technician responsibilities with 50% patient care coordination, including chronic disease support, medication synchronization (Med Sync), remote patient monitoring, wellness program development, and proactive patient outreach.

In this role, you'll serve as a trusted partner for patients by coordinating care, supporting medication adherence, promoting preventive wellness, and helping individuals successfully manage chronic conditions.

Core Responsibilities

Certified Pharmacy Technician Responsibilities (~ 50%)

• Prescription data entry and processing

Patient Care Coordination Responsibilities (~ 50%)

A Patient Care Coordinator serves as the quarterback for a defined cohort of patients, coordinating their care journey, connecting them with CCRX pharmacy, wellness, and clinical services, and helping them achieve better health outcomes through proactive engagement, accountability, and personalized support. You will be entrusted with a panel of patients and play a key role in improving engagement, adherence, outcomes, and overall participation in the CCRX care model.

Responsibilities include:

• Managing Med Sync, medication adherence, and patient engagement programs
• Coordinating chronic condition support, including diabetes care and remote patient monitoring (RPM)
• Identifying care gaps and opportunities for intervention related to medications, preventive care, wellness services, metabolic health, and pharmacist-led clinical programs
• Preparing and coordinating wellness consultations, follow-up care, and patient care plans
• Connecting patients with appropriate CCRX services, products, programs, and resources
• Tracking patient outcomes, documenting progress, and supporting communication among patients, providers, pharmacists, and the care team

Compensation

• Entry-level tech: $18 per hour
• Nationally Certified Pharmacy Technician: Base Compensation: $20 -21 per hour

Professional Development Incentives

• Additional $1.00 / hour: vaccination certification
• Additional compensation per hour: completion of another advanced certification (can stack multiple certifications that are approved by the pharmacy owner)
• CCRX will cover the cost of approved certification and training programs. Employees who voluntarily separate employment within 12 months of completing employer-funded certification may be required to reimburse CCRX for the training costs.

Performance Incentive Opportunity: Eligible to earn additional compensation through performance-based incentives tied to outcomes that benefit both patients and the pharmacy. As this is a new role, many of these performance measures will be developed and refined collaboratively as the program grows. Potential incentive categories may include:

Our objective is for a strong performer to average approximately $5.00 per hour in additional incentive compensation over the course of a year.

Potential compensation range:

• Entry Level: $18-20.00/hour
• Certified & High Performing: Target annualized compensation of approximately $25.00/hour

Ideal Candidate

• Certified Pharmacy Technician or equivalent
• Compounding experience
• Immunization certified
• Interested in chronic disease management and wellness services
• Interest in pursuing advanced certifications (e.g., diabetes education, compounding, point-of-care testing, and body composition analysis)
• Strong communication and relationship-building skills
• Passion for helping patients improve their health
• Organized, proactive, and self-motivated
• Comfortable and diligent with patient outreach and follow-up
• Excited to co-create, build and improve new programs
• Comfortable with technology and eager to leverage new tools, automation, and emerging technologies to improve patient care and operational efficiency

Schedule

• Full and part time positions available (~20-40 hours per week)
• Typical shifts range from 8.5–10 hours between 7:00 AM and 6:30 PM, based on pharmacy operations and patient care programming needs
• Rotating Saturdays (8:30 AM–3:00 PM or 8:30 AM–1:00 PM seasonally)
• Occasional evening or weekend participation in wellness events, educational programs, or community outreach activities may be required
• Potential future hybrid or remote work opportunities may be available as care coordination workflows mature and performance expectations are consistently achieved

Reporting Structure & Accountability

The Patient Care Coordinator reports directly to the Pharmacy Owner and works under the clinical guidance of the Pharmacist-in-Charge (PIC). This role collaborates closely with pharmacists, technicians, wellness providers, and other members of the healthcare team.

Because this position is responsible for developing and managing new patient care initiatives, it requires a high degree of independence, initiative, resourcefulness, and personal accountability. The Patient Care Coordinator will be entrusted with specific programs, patient populations, projects, and performance goals and will be expected to manage responsibilities proactively while meeting established timelines, deliverables, and performance expectations.

Success in this role requires strong organizational skills, effective communication, and the ability to identify opportunities, solve problems, and move initiatives forward with minimal supervision.
