This is an opportunity for an Illinois Licensed Social Worker (LSW) or Illinois Licensed Professional Counselor (LPC) to lead an Assertive Community Treatment (ACT) team providing a very specialized model of treatment/service delivery. A multi-disciplinary team serves a small, defined caseload of seriously mentally ill (SMI) adults and becomes the single point of responsibility for that caseload. This is a unique treatment model in which the majority of mental health services are directly provided by the ACT team in the client's regular environment.
Team Lead Key Duties:
- Lead the ACT team consisting of a Psychiatric Mental Health Nurse Practitioner, Addictions Counselor, Nurse Practitioner, Case Manager, Peer Specialist, and Administrative Assistant to coordinate activities and ensure each client receives the highest level of service
- Create policies, procedures, and a program manual for the newly forming ACT team
- As a team member, provide direct service to ACT clients in their homes / places of residence
- Conduct mental health assessments and reassessments using IM CANS
- Develop treatment plans using IM CANS
- Conduct therapy and counseling for each client
- Monitor progress of the treatment plan for each client and update as needed
- Conduct on-site a daily one-hour meeting, at least four days per week, with the ACT team
- Document services and submit billing information in system of electronic health records on same day of service
- Respond to occasional after-hours or weekend phone calls and emergencies
- Know and work within federal and state rules, laws, and regulations. Ensure compliance with Joint Commission, SAMHSA, HFS, IDHS, and other requirements.
- Other duties as assigned by the Chief Program Officer
Team Lead Requirements:
- A master’s degree in social work, counseling, psychology or a related field giving knowledge of human behavior
- Current Illinois license as either a Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC)
- At least four years of management experience
- Either a current IM CANS certification or earn the certification within 30 days of hire
- Computer literate and experienced with electronic health records

Pay: $35.00 per hour
