What are the responsibilities and job description for the Crisis Case Manager position at Comprehend Inc?
Role and Responsibilities
Assists clients served by the Crisis Team in gaining needed access to medical, social, educational, financial, and other community support services within and outside the agency.
Serve as a member of the multidisciplinary Crisis Team.
Participate in daily team staffing and treatment plan review meetings.
Provide follow-up of clients served by Crisis Team Attend staff meetings, in-service trainings and workshops as required.
Advocates for, coordinates, and links clients with needed services and resources.
Identifies barriers for accessing services and a plan to address them and ensure outpatient follow up care.
Complete all documentation in a timely and accurate manner in accordance with medical records guidelines.
Capable of visiting consumers in their own home which may not be handicapped accessible.
Other duties may be assigned.
Qualifications and Education Requirements
Bachelor of Arts or Science Degree in any of the behavioral sciences from accredited institution.
Behavioral Sciences include psychology, social work, sociology, counseling and human services.
One year of experience in performing case management services or working with behavioral health population.
Master’s degree in behavioral science may substitute for the one year of experience.
Special Requirements
Must possess a valid driver’s license.
