- Clinical Care: Administer medications to residents safely and accurately in accordance with their specific care plans.
- Physician Orders: Perform treatments as assigned per physician’s orders, always staying within the LPN scope of practice.
- Resident Monitoring: Carefully observe, report, and record resident conditions. This includes identifying signs and symptoms that may indicate a change in status and communicating those changes to the RN or Unit Manager.
- Documentation: Maintain precise and timely medical records to ensure continuity of care across all shifts.
- Regulatory Compliance: Perform all duties in strict accordance with local, state, and federal guidelines, as well as facility policies.
- Collaborative Duties: Support the Unit Manager, Director of Nursing, and Administrator with additional tasks as assigned to ensure a high standard of facility operation.
- Education & Licensure: Must hold an active Licensed Practical Nurse (LPN) license and be eligible to work in the state of Wisconsin.
- Technical Skills: Proficiency with computers and Electronic Health Records (EHR) is required.
- Professionalism: Ability to work effectively as part of a multidisciplinary team or independently when required.
- Experience: Previous experience in a long-term care (LTC) setting is preferred, though we welcome all qualified applicants who possess a heart for senior care.
