- Clinical Execution: Prepare and administer medications and perform treatments as ordered by physicians, ensuring strict adherence to the LPN scope of practice in Wisconsin.
- Proactive Monitoring: Conduct thorough physical and emotional assessments throughout your shift, identifying and reporting subtle status changes to the RN or DON to prevent hospitalizations.
- Precise Documentation: Maintain accurate, real-time electronic records of resident care and clinical interventions in compliance with state and federal regulations.
- Order Management: Transcribe physician orders to MARs/TARs with meticulous attention to detail and coordinate directly with the pharmacy for new prescriptions.
- Unit Leadership: Supervise and mentor Certified Nursing Assistants (CNAs), ensuring that personal care is delivered with dignity and meets our high standards.
- Collaborative Care: Participate in interdisciplinary team meetings to provide clinical insights for individualized resident care plans.
- Licensure: Must possess an active and unrestricted Licensed Practical Nurse (LPN) license in the state of Wisconsin.
- Clinical Judgment: Strong critical thinking skills and the ability to remain calm and effective in a fast-paced environment.
- Technical Skills: Basic computer proficiency for electronic charting (EHR) and strong organizational habits.
- Experience: Previous experience in long-term care or sub-acute settings is a plus, but highly motivated new graduates are encouraged to apply! We offer a robust orientation program.
