Complete Care Management is seeking an experienced External Case Manager to support our skilled nursing facilities across the Wisconsin region. This regional, field-based role partners closely with hospital systems, payors, facility leadership, and internal care teams to ensure safe transitions, reduce avoidable hospitalizations, and optimize length of stay and reimbursement outcomes.The ideal candidate brings strong clinical knowledge, exceptional communication skills, and experience navigating post-acute care coordination within skilled nursing environments.Key ResponsibilitiesServe as a regional liaison between Complete Care facilities, hospitals, payors, and community providersCoordinate and manage transitions of care from acute hospitals to skilled nursing facilitiesCollaborate with facility interdisciplinary teams to support discharge planning and care progressionMonitor patient status, barriers to discharge, and opportunities to reduce length of stayAssist with authorization processes and communicate with insurance providers as neededSupport facilities with readmission prevention strategies and care coordination best practicesParticipate in case reviews, care conferences, and meetings with hospital partnersProvide education and guidance to facility teams on external case management processesTravel regularly to assigned facilities throughout the Wisconsin regionQualificationsPrior experience in case management, care coordination, discharge planning, or utilization managementSkilled nursing facility or post-acute care experience strongly preferredStrong understanding of payor requirements, authorizations, and care transitionsExcellent communication, organization, and relationship-building skillsAbility to work independently in a regional, field-based roleValid driver’s license and ability to travel throughout WisconsinWhy Join Complete Care?Mission-driven organization focused on quality outcomes and patient-centered careOpportunity to make a regional impact across multiple skilled nursing facilitiesCollaborative leadership team and strong operational supportGrowth-oriented organization with long-term stability
