Key Responsibilities:
- Ensure accurate and timely documentation in compliance with state and federal regulations
- Participate in interdisciplinary care plan meetings to support residents’ social and emotional well-being
- Educate residents and families regarding Advanced Directives
- Coordinate and facilitate smooth admissions and discharges
- Support residents and families through transition planning and ongoing care needs
Qualifications:
- Current New Jersey LSW, CSW, or LCSW required
- Prior staff management experience preferred
- Long-term care experience a plus
- Strong communication and organizational skills
- Ability to work both independently and collaboratively within an interdisciplinary team
