We are seeking a highly skilled and motivated Delegating Nurse/Case Manager to coordinate comprehensive patient care within various healthcare settings. The ideal candidate will possess extensive clinical expertise, including critical care, trauma, and hospital medicine, along with strong case management skills. This role involves overseeing patient assessments, discharge planning, and utilization management to ensure optimal outcomes and seamless care transitions.
- Conduct thorough patient assessments, including vital signs, physiological evaluations, and medical history reviews, to develop individualized care plans.
- Delegate nursing tasks appropriately to licensed and unlicensed staff while ensuring adherence to regulatory standards such as HIPAA.
- Coordinate inpatient and outpatient services across hospital, clinic, and post-acute care environments, including PICU (Pediatric Intensive Care Unit) and Geriatrics settings.
- Manage discharge planning processes by collaborating with multidisciplinary teams to facilitate smooth transitions from hospital to home or other care facilities.
- Oversee diagnostic evaluations, medical records documentation, and utilization management to optimize resource use and patient outcomes.
- Provide clinical guidance in emergency room situations, trauma centers (Level I & II), infectious disease care, and behavioral health cases.
- Ensure compliance with healthcare regulations by maintaining accurate medical records and adhering to medical terminology standards.
- Support patient service initiatives through effective communication, intake procedures, and comprehensive case management strategies.
- Proven experience in critical care environments such as ICU (Intensive Care Unit), emergency medicine, or trauma centers (Level I & II).
- Demonstrated expertise in hospital medicine, medical-surgical units, outpatient clinics, or post-acute care settings.
- Strong background in case management with knowledge of utilization management and discharge planning processes.
- Familiarity with pediatric (PICU), geriatrics, behavioral health, infectious disease care, and physiologic assessment techniques.
- Excellent understanding of medical terminology, HIPAA regulations, and medical records documentation standards.
- Ability to perform detailed patient assessments with proficiency in vital signs monitoring and diagnostic evaluation.

Pay: $34.80 - $60.00 per hour
