General Description of Duties:
They shall provide nursing services that include assessing individual’s health needs, developing care
plans, visiting homes and supervising direct support professionals in implementing delegated nursing tasks and medication administration. In addition, acts as a liaison with physicians and other healthcare professionals, maintains appropriate nursing documentation including 45-day reviews, provides staff training, and participates as an integral member of each individuals support team.
Essential Tasks and Responsibilities
- Makes initial and ongoing nursing assessments, judgments and treatment decisions based on individual and family/caregiver needs and wishes and in accordance with her/his professional skills.
- Shares on-call responsibilities
- Assess new intakes of people we support
- Update and revise individual’s health care plans as needed
- Educate individuals and their families about health care plans and options
- Make clinical decisions for routine individual receiving services care
- Accurately document and submit medical records
- Demonstrate knowledge in medical care, such as diagnostic procedures, medication, symptoms, and other treatment-related therapies
- Provide emotional support to people we support and their families
- Evaluate and determine needs of individuals receiving services
- Review results from medical tests
- Conduct comprehensive assessment of individuals physical, mental, and psychosocial needs
- Utilize behavioral strategies help individuals adopt healthy behaviors and improve self-care in chronic disease management.
- Promote self-management goals.
- Assist individuals receiving services in navigating the health care system.
- Coordinate Specialty care, follow-up on test results and other care coordination needs.
- Follow-up with individuals within 24 hours on any medical discharges and within 48 hours with respective Program Managers
- Work effectively and cooperatively with the programming team to ensure individuals receiving services are provided the best quality of care.
- Partner with external case management programs to coordinate care
- Ongoing evaluation and documentation of individual progress/ risk status Document in Carematic; communicate with programing team
- Implement physician orders and other nursing procedures
- Create an environment for open communication
- Other duties as assigned.
Qualifications:
- Valid Maryland RN license.
- DDA-Delegating Nurse experience required with Active Status with DDA as a Delegating Nurse.
- MTTP Trainer experience preferred
- Effective ability to work well under pressure, Excellent organizational skills and ability to prioritize and handle multiple tasks at once and ensure proper follow through as needed.
- Effective problem-solving skills
Education
- BSN or Bachelor’s degree in related field and at least two years’ experience working in human services preferably with
persons with intellectual disabilities - Excellent written and verbal communication skills.
Certifications
- Must have valid Motor Vehicle Class “C” Driver’s License and access to reliable transportation.
- Maintenance of all required training's under DDA and CMT guidelines and Langton Green’s.
Physical Demands:
- While performing the tasks of this position, the employee is required to stand, walk, sit, bend, kneel, and use hands to finger, handle or feel, reach with arms and hands, balance, lift, stretch.
- Must be capable of lifting/moving over 50 pounds and the transferring of individuals with or without equipment.
