Compassionate Home Health Care, Inc. is seeking compassionate and dependable Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide skilled nursing care to a client in their home in Farrell, PA.

Position Responsibilities:
- Complete nursing assessments and monitor client health conditions
- Document patient care and maintain accurate electronic records
- Administer medications and provide medication management
- Perform vital sign checks and clinical evaluations
- Provide wound care and monitor healing progress
- Educate clients and families on home care procedures
- Communicate with physicians, therapists, and other members of the care team
- Coordinate services according to the client’s individualized plan of care
- Promote safety, comfort, and independence in the client’s home
Paid Training Provided
All new employees complete paid on-site training covering:
- Medication administration
- Documentation and charting
- Patient assessments
- Daily care procedures
- Electronic Visit Verification (EVV)
- Home health nursing expectations
Qualifications:
- Current, active Pennsylvania LPN or RN license
- Valid Driver’s License
- Willingness to work some weekends
- Ability to pass background check and drug screening
- Basic computer skills and ability to use electronic medical record (EMR) systems
- Ability to use EVV system through a mobile application
Preferred Experience:
- Home health experience
- Ventilator/tracheostomy experience (preferred, but training available)
- Strong clinical assessment and documentation skills
