Responsibilities

- Provide memory care support to individuals experiencing cognitive challenges.
- Facilitate community engagement by accompanying clients on outings and activities.
- Maintain a clean and safe living environment for clients by performing light housekeeping tasks.
- Monitor and document vital signs as needed, reporting any changes to the appropriate healthcare professionals.
- Support clients in developing life skills and achieving personal goals through individualized care plans.
- Lift and assist clients as necessary to ensure their safety and comfort during daily activities.
- Collaborate with a multidisciplinary team, including nurses and other healthcare providers, to deliver comprehensive care.
