LPN Case Manager

The LPN Case Manager is a Licensed Practical Nurse that provides indirect patient care services and manages patient care in accordance with policies and procedures of Compassion Home Care, Inc. Must have Oklahoma or Compact State LPN License in good standing with OBN.

RESPONSIBILITIES AND DUTIES:

• Under direct supervision of Clinical Director
• Reviews POC developed by RN/Clinical Director.
• Continually re-evaluates patient needs and updates POC as needed.
• Notifies all disciplines involved in patient care of orders on POC.
• Keeps track of patient encounters in order to maintain correct frequencies.
• Develops and provides SN schedules on weekly basis.
• Attends scheduled case conferences and develops written documentation if case conference to develop POC.
• Communicates frequently with all disciplines involved inpatient care in order to provide quality and continuity of care to patients.
• Reviews and co-signs all orders and provides copies of orders for all disciplines involved.
• Handles all incoming calls regarding patients under Case Manager’s care.
• Notifies physician of any abnormalities reported by field staff as needed.
• Notifies patient/family of changes in plan of care as they occur as applicable.
• Is available to all disciplines involved in patient care during business hours in case of problems or questions concerning patient.
• Performs other duties in absence of or assistance to other staff, as requested.
• Maintains confidentiality of all patient records and office information.
• Required to participate in an on-call rotation.
