Physical Therapist (PT)
$45–$50/hour | 1099
Compass Palliative Care is seeking a compassionate and experienced Physical Therapist to provide in-person care to patients in Skilled Nursing and Assisted Living Facilities.
Responsibilities
- Provide physical therapy evaluations, treatments, and individualized care plans focused on mobility, strength, balance, pain management, and quality of life
- Monitor patient progress, maintain clinical documentation, and collaborate with facility staff and the healthcare team
Requirements
- Active and unrestricted California Physical Therapist license
- Graduate of an accredited Physical Therapy program
- Experience with geriatric, medically complex, or chronic-care patients preferred
- Strong communication skills and ability to work independently
- Reliable transportation for in-person visits
Position Details
