- Perform initial evaluations and develop individualized Plans of Care
- Provide skilled nursing services including wound care, IV therapy, medication management, and patient education
- Regularly re-evaluate patient needs and revise care plans as needed
- Coordinate with physicians, therapists, and other disciplines for seamless care
- Supervise and teach Home Health Aides
- Maintain accurate, timely documentation and ensure compliance with all regulations
- Participate in quality improvement as required
- Current Oklahoma Registered Nurse license
- Minimum 1 year of clinical experience (home health, med/surg, or similar preferred)
- Strong assessment, communication, and problem-solving skills
- Current CPR certification
- Valid driver’s license, reliable transportation, and current auto insurance
- $2,500 Sign-On Bonus paid out over the first 6 months
