What You’ll Do
- Collaborate with a multidisciplinary team to manage patient populations with chronic health conditions.
- Assess patients and develop individualized care plans for the care coordination program.
- Implement and evaluate care plans to ensure optimal health outcomes.
- Educate patients on disease processes, medications, and self-care strategies.
- Support patients through transitions of care to improve outcomes.
What We’re Looking For
Required:
- Current NC Registered nurse license
- Current NC driver’s license
- BLS Certificate from American Red Cross or American Heart Association
Preferred:
- Two (2) years of experience in primary healthcare
- Ambulatory care and/or home health care within the past five (5) years.
- Evidence of continuing education in health care. An equivalent combination of related education and experience may be considered.
