What are the responsibilities and job description for the Smoking Cessation and Navigator Coordinator position at CommUnityCare Health Centers?
In collaboration with patients, families (as defined by patient) and staff across all disciplines and departments, Lung Cancer Screening and Smoking Cessation Patient Navigator will work for a lung cancer screening and smoking cessation program, funded by the Cancer Prevention Research Institute of Texas (CPRIT), to increase access to Lung Cancer Screening and smoking cessation services for former and current smokers. This person is responsible for supporting the goals of the program and applying empowerment-based prevention and intervention strategies to improve community health specifically related to lung cancer screening, smoking cessation services, making referrals to community-based resources, addressing barriers to improving well-being, offering expertise regarding psycho-social issues affecting patient care, and educating patients regarding illness and treatment options.

Essential Functions
- Apply empowerment-based prevention and intervention strategies to improve community health and facilitate smoking cessation education, low dose CT (LDCT) screening, follow-up and treatment.
- Lead training for CUC providers and staff on lung cancer screening criteria and techniques for increasing referrals to intensive smoking cessation services.
- Organize program mailings to eligible patients
- Respond to participating patients for access to intensive smoking cessation services and/or lung cancer screening
- Provide intensive smoking cessation services
- Facilitate shared decision making about lung cancer screening
- Assess, diagnose, and treat smoking disorders as well as comorbid mental health disorders (if LMSW, under the supervision of a board-certified LCSW)
- Ensure the timely receipt of screening and timely return of results
- Help patients requiring additional testing or treatment through patient navigation
- Participate in all team meetings and contribute to presentations and publications.
- Assist with data management related to lung cancer screening and smoking cessation services, including tracking of completed tests and management of testing databases.
- Provide accurate and complete documentation of all patient contacts and patient progress towards LDCT completion.
- Address personal and environmental issues with the patient in order to maximize emotional, social, and physical wellbeing and the appropriate use of healthcare resources.
- Help patients and families understand lung cancer screening, test results, follow-up procedures and treatment options, as well as consequences of various treatments or treatment refusal.
- Communicate and collaborate with clinic providers and support staff to assess patient’s needs and provide comprehensive healthcare.
- Complete all direct and indirect care documentation in timely manner and ensure patient records are current and complete.
- Collaborate and communicate with the University of Texas Dell Medical School Department of Medicine Program Coordinator.
- Attend seminars and maintain all licensure and/or certification requirements for continuing education and bestpractices.
- Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare
- Standard Operating Procedures and CommUnityCare Policies and Procedures.
- Develop and maintain favorable internal relationships, partnerships with co-workers, including clinical managers, clinical support staff, providers and business office staff.
- Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families.

- Bachelor's Degree (higher degree accepted) Psychology, Public Health, RN Degree or closely related field
- 1 year social work experience
- LMSW - Licensed Medical Social Worker - State Licensure in the State of Texas Upon Hire Required Or
- LCSW- License Clinical Social Worker in the State of Texas Upon Hire Required
