Nurse Navigator

Everguide Health
Austin, TX Full Time
POSTED ON 3/10/2026
AVAILABLE BEFORE 5/4/2026

About Everguide HealthEverguide Health practices medicine as a relationship. We care for a small, intentional panel so we can know our members well, show up when it matters, and practice medicine the way it was meant to be practiced. We are a family medicine led concierge practice built on private health stewardship- partnering with individuals and multi-generational families to guide health over time. We provide comprehensive primary care for adults and children, emphasizing continuity, proactive planning, and thoughtful coordination across every stage of life. Our model prioritizes depth, trust, and long-term outcomes, supported by a dedicated care pod designed to protect both the patient experience and physician well-being.

What You Will Do

The Registered Nurse serves as the clinical backbone of the care pod. You will translate physician direction into action, ensure follow-through across diagnostics and care plans, and act as a consistent point of contact for members. You are not only delivering care—you are stewarding it.

Clinical Care & Diagnostics

  • Perform assessments, phlebotomy (adult and pediatric), point-of-care testing, and medication administration.
  • Monitor and document clinical data accurately in the EHR.
  • Travel locally to meet members in their homes, offices, and other appropriate community settings.
  • Be comfortable entering members’ homes and work environments as a trusted clinical professional.
  • Maintain professionalism, situational awareness, and clinical standards in non-traditional care settings.
  • Adapt workflows while preserving safety, compliance, and member experience.
  • Communicate results clearly and efficiently, escalating when appropriate.
  • Coordinate with specialists and external providers to maintain continuity of care.
  • Anticipate needs and close loops without being prompted.

Member Advocacy & Operational Excellence

  • Reinforce preventive strategies and long-term health plans.
  • Advocate for members within the healthcare system when coordination or access is required.
  • Participate in developing and refining clinical workflows and policies.
  • Protect confidentiality and uphold the highest professional standards.

Who This Role Is For

This role is for a nurse who:

  • Thinks longitudinally, not episodically.
  • Wants to practice medicine beyond the four walls of a clinic.
  • Is comfortable and confident providing care in members’ homes and workplaces.
  • Is adaptable, professional, and composed in varied environments.
  • Communicates clearly and takes ownership of outcomes.

What We’re Looking For

  • Active Texas RN license.
  • Associate’s or Bachelor’s degree in Nursing.
  • Minimum 4 years of clinical experience (inpatient or outpatient).
  • Strong phlebotomy skills (adult and pediatric required).
  • Experience in care coordination or concierge-style settings preferred.
  • CPR certification.

Work Structure & Expectations

  • Full-time (40 hours per week).
  • Competitive salary based on experience and alignment.
  • Paid time off and holidays.
  • Opportunities for professional growth within an expanding care model.
