What You Will Do
The Registered Nurse serves as the clinical backbone of the care pod. You will translate physician direction into action, ensure follow-through across diagnostics and care plans, and act as a consistent point of contact for members. You are not only delivering care—you are stewarding it.
Clinical Care & Diagnostics
- Perform assessments, phlebotomy (adult and pediatric), point-of-care testing, and medication administration.
- Monitor and document clinical data accurately in the EHR.
- Travel locally to meet members in their homes, offices, and other appropriate community settings.
- Be comfortable entering members’ homes and work environments as a trusted clinical professional.
- Maintain professionalism, situational awareness, and clinical standards in non-traditional care settings.
- Adapt workflows while preserving safety, compliance, and member experience.
- Communicate results clearly and efficiently, escalating when appropriate.
- Coordinate with specialists and external providers to maintain continuity of care.
- Anticipate needs and close loops without being prompted.
Member Advocacy & Operational Excellence
- Reinforce preventive strategies and long-term health plans.
- Advocate for members within the healthcare system when coordination or access is required.
- Participate in developing and refining clinical workflows and policies.
- Protect confidentiality and uphold the highest professional standards.

What We’re Looking For
- Active Texas RN license.
- Associate’s or Bachelor’s degree in Nursing.
- Minimum 4 years of clinical experience (inpatient or outpatient).
- Strong phlebotomy skills (adult and pediatric required).
- Experience in care coordination or concierge-style settings preferred.
- CPR certification.
