This position has the flexibility to create your own hours based on the needs of the department. Mon-Fri - No Weekends - No Holidays
You can choose five 8 hour days, four 10 hour days, or 3 12 hours days per week based on the needs of the program.
This employee will be eligible for mileage re-imbursement

Main Function: The Home Care RN Supervisor is responsible for providing clinical oversight of home care services to ensure safe, effective, and compliant care. The RN Supervisor conducts client assessments, supervises Personal Care Aides (PCAs), monitors care plans, maintains required clinical documentation, and collaborates with clients, families, physicians, and agency staff to promote quality care and positive client outcomes. The Home Care RN Supervisor is also responsible for the direction and supervision of all health care services and programs of the Licensed Home Care Agency (LHCSA) Assisted Living Program.

Duties and Responsibilities:

  • Conduct initial and ongoing client assessments to determine eligibility for home care services and ensure safe, appropriate care can be provided.
  • Perform Uniform Assessment System for New York (UAS-NY) assessments and reassessments as required by New York State regulations and agency policy, including completion of monthly UAS assessments for eligible clients, and ensure documentation is accurate, timely, and supports the client's plan of care for the Assisted Living Program residents.
  • Admit only those clients for whom safe and adequate care can be rendered.
  • Perform supervisory visits for each client at least every 90 days, or more frequently as required by agency policy or regulation, to evaluate aide performance and ensure the plan of care is being followed.
  • Develop, review, and update individualized plans of care based on client needs and physician orders.
  • Evaluate Home Health Aide performance through supervisory visits and provide guidance, education, and corrective instruction as needed.
  • Ensure staff assignments are appropriate based on client needs, aide training, competency, and experience.
  • Collaborate with scheduling staff to promote continuity of care whenever possible.
  • Maintain accurate and complete client clinical records in accordance with agency policy and regulatory requirements.
  • Review Home Health Aide documentation and activity sheets for accuracy and compliance prior to billing.
  • Maintain referrals, physician orders, authorizations, and other required documentation for contracted and private pay clients.
  • Ensure each client is informed of Patient Rights and Responsibilities upon admission.
  • Communicate and collaborate with physicians, nurse practitioners, and other healthcare providers regarding client status, changes in condition, and care needs.
  • Maintain regular communication with clients and their families regarding care needs, concerns, and service coordination.
  • Participate in Quality Assurance and Performance Improvement activities as assigned.
  • Assist with orientation, training, and ongoing education of Home Health Aides and Personal Care Aides as directed.
  • Investigate and document client concerns or complaints related to nursing or aide services and assist in resolution.
  • Provide medication setup or medication management services for private pay clients as ordered by the client's provider and permitted under agency policy.
  • Perform other clinical duties as assigned by the VP of Senior Housing Services.

Education:

  • Graduate of an accredited school of nursing.
  • Current New York State Registered Nurse (RN) license in good standing.
  • Current CPR certification preferred.

Qualifications:

  • Minimum of one year of home care, community health, or geriatric nursing experience preferred.
  • Knowledge of New York State Licensed Home Care Services Agency (LHCSA) regulations and standards preferred.
  • Demonstrated ability to assess clients, develop care plans, and supervise Personal Care Aides.
  • Strong clinical judgment and organizational skills.
  • Excellent verbal and written communication skills.
  • Ability to establish effective working relationships with clients, families, physicians, and interdisciplinary team members.
  • Proficiency with electronic health records and clinical documentation systems.
  • Valid driver's license and reliable transportation for community visits.
