Education/Experience:
- Current NJ RN License
- Valid Driver's License required
- One year exp. in home health preferred
Job Summary: Provide direct care and patient teaching related to disease process
Job Description:
- Monitor and assess patients health conditions, documenting changes and progress
- Modify care plan as indicated according to patients response and needs
- Administer treatments as prescribed by health care provider
- Educate patients and families on health management and disease prevention
- Collaborate with interdisciplinary team to develop and implement care plans
